Welcome New Patients

We are so glad you are here! At Kinetic Physical Therapy, all appointments are scheduled for 55 minutes, 1:1 with your Physical Therapist. We will look comprehensively at your posture, movement, flexibility, strength, activities and more. Please wear clothing that is flexible and allows for easy examination of the affected area and surrounding areas.

To expedite your first appointment, intake forms will be sent to you via email. Please complete them prior to your appointment. If you prefer paper forms, you can print New Patient Intake Forms and bring them with you to your first appointment along with your Physical Therapy prescription, photo ID either your insurance card(s) or form of payment for services. Please also take time to review the privacy practices.

We are currently In Network/Preferred providers for:

• Medicare (traditional plans)
• PIP
• L & I/Workers Comp
• Regence (through July 2024)
• Blue Cross/Blue Shield (Regence based plans through July 2024)

We are currently Out of Network providers for:

• Premera
• Blue Cross/Blue Shield (Premera based plans)
• Aetna
• Kaiser PPO
• First Choice
• United Health Care

If we are out-of-network with your insurance company, we offer a 32% discount for 55 minute appointments and can supply you with an invoice or superbill for insurance reimbursement upon your request.

Payment is accepted in forms of cash, check, HSA, FSA, debit & credit cards. There is a 3% service charge for credit cards.
